GRANT REPORT GUIDELINES

The directors and staff of the Sam L. Cohen Foundation maintain a continued interest in the organizations and projects we support. Your report enables us to review our grant-making programs and evaluate more effectively the impact of our grants.

In your grant report, please address the following questions:

  1. How were the funds from the grant used? Were all the grant monies spent?
  2. What impacts resulted from the grant or project? Were the objectives achieved?
  3. What worked well? What were the challenges? What did you learn?
  4. What is the current status of the project? Will it continue?

In addition, provide a project budget with actual costs.

Please limit your report to three pages or fewer. If possible, provide your report electronically. Feel free to contact the Sam L. Cohen Foundation staff if you have questions.
